Il bambino tanto atteso da Katherine Torn (Lee Remick) muore alla nascita. Il marito, l’ambasciatore italiano a Roma Robert Thorn (Gregory Peck), non sa come dirglielo. Su consiglio di Padre Spiletto (Martin Benson), direttore dell’ospedale, e senza dir nulla alla moglie, Robert sostituisce il figlio morto con un altro bambino (nato la stessa notte) la cui madre è morta di parto. Subito dopo, Robert, nominato ambasciatore in Gran Bretagna, si trasferisce con la famiglia a Londra. La vita scorre serena e il figlio Damien (Harvey Stephens) cresce, ma alla festa del suo quinto compleanno la sua bambinaia si impicca davanti a tutti dicendo che lo fa per lui.
